Frontier Home Health + Hospice provides home health and hospice services to adults and seniors throughout the Dallas–Fort Worth area and surrounding communities. Home health focuses on recovery, rehabilitation, and management of medical needs through skilled nursing, physical and occupational therapy, medical social services, home health aides, wound care, IV care, and other clinical services. Hospice provides comfort-focused care for people with life-limiting illnesses and support for their families. Frontier states that its care planning is guided by each patient's goals, preferences, autonomy, and choices. Services are primarily delivered where patients live, although hospice may also be provided in nursing homes, hospitals, and other long-term-care settings.
